Chino Valley Medical Center - Life Stream Blood Drive
Please help save a life by coming and donating blood on Friday, February 16th from 10am - 3pm in the Conference Center.
For every lifesaving donation participants will receive: a coupon for two complimentary tacos, courtesy of Jack in the Box (while supplies last), free cholesterol screening, and points that may be redeemed for items through LifeStream’s online donor store at www.LStream.org.
Make sure to drink plenty of fluids and bring your photo id. If you are 15 or 16 years old, make sure you bring a signed LifeStream parental consent form. Walk-ins are welcome but appointments are strongly recommended and will receive priority
